Plans are required by law to collect the taxpayer identification number (TIN) or Social Security number (SSN) of each medical plan participant and provide that number on reports that will be provided to the IRS each year. Moreover, plans are required to make at least two consecutive attempts to gather missing TINs/SSNs. If someone has a dependent who does not yet have an SSN, they can go to the government’s social security website to complete a form to apply for a SSN free of charge.
